The Century-Long Silence
For over a century, the field of neurodegeneration research has been defined as much by its divisions as by its discoveries. Since 1906, when Alois Alzheimer's description of "presenile dementia" captured the scientific imagination, the narrative has often narrowed around singular pathologies — most notably the amyloid cascade hypothesis. That focus generated invaluable data. It also created deep silos. Virologists studying HSV-1 in Alzheimer's rarely converse with lysosomal biologists; geneticists identifying PSEN1 mutations often work in isolation from environmental toxicologists.
The silos are not abstract. They can be named, and they map almost exactly onto the three phases this corpus describes:

Three communities, three literatures, one disease — and very few citations crossing between them.
The erasure
The history of this science is also fraught with what is best called political erasure. As the analysis of early twentieth-century neuropathology in this corpus shows, the contributions of Oskar Fischer — who described the "inside-out" formation of plaques and their drusiform (gland-like) necrosis — were largely sidelined in favour of the Munich School's amyloid-centric model. The narrowing had a cost: a paradigm that accurately described the cellular mechanism of dementia was effectively lost for nearly a hundred years.
- 1907 — Fischer publishes Sphaerotrichia cerebri multiplex; the framework is eclipsed by Munich.
- 2007 — Nixon characterises PANTHOS; the lysosomal-acidification mechanism is revealed.
- 2026 — The tripartite architecture is synthesised; the Phase I → II → III drug landscape is mapped.
The Century-Long Silence is not a metaphor. It is the absence of citations that should exist between Fischer's brainstem observations, Stevens's complement-mediated synapse pruning, and Fawcett's perineuronal-net biochemistry. Naming that absence is a load-bearing act, because the same disciplinary fragmentation that delayed the synthesis still shapes how the next generation of trials is designed.

Why this site exists
Adult Cognitive Disease was founded to correct that fragmentation. The premise is that the answers to neurodegeneration are already present in the scientific literature — they are simply disconnected, buried across millions of papers and spanning disciplines that speak different technical languages.
This wiki is the corpus that follows from the premise: every paper, every concept, and every link between them, rendered so the connections are visible rather than implied. The seven monographs are the argument those papers add up to, written as continuous narrative.
